HOW TO REPORT HAZARDS IN THE WORKPLACE
Employees are encouraged to report safety hazards to their supervisor by submitting a “Report of Safety Hazard” form. It is the responsibility of the supervisor to ensure all safety hazards reported are documented.
The supervisor is responsible for notifying the Department Safety Representative, so that an investigation of the matter can commence and appropriate action can be taken to abate the hazard.
The Department Head shall be notified if the safety hazard cannot be corrected immediately and the hazard is a serious threat to the health and well being of county employees and the public.
A safety hazard may also be reported directly to the Risk Management/Safety Office by calling 538-7631 or 538-7173, however, this is recommended only after responsible persons in the department have failed to correct the safety hazard.
Reports of safety and health hazards may also be made directly to Cal-OSHA by calling (530) 224-4743. Employees are encouraged to allow supervisors and department safety representatives ample time to resolve health and safety problems before calling Cal-OSHA.
This notice shall be posted on safety bulletin boards explaining the procedures for reporting safety hazards in the workplace. The department shall provide the “Report of Safety Hazard” forms to employees so that hazards are communicated to responsible persons. The forms also serve as documentation for recordkeeping purposes.
There shall be no reprisal to any employee who makes a complaint to their supervisor, department safety representative, the Butte County Risk Management/Safety Office, or Cal-OSHA.
